What are custom orthotics?
Custom orthotics are medical devices prescribed by a foot and ankle surgeon to support areas in your feet that may be causing you problems. They are different from shoe inserts and arch supports in that they serve these functions:

Accommodate or offload a prominent area. This can be helpful for ulcers, calluses beneath bony prominences, and bursitis (inflammation of the fluid-filled sacs that act as cushions at your joints).

Help the foot function better in the case of hypermobility. Beneficial for those suffering from tendonitis, fasciitis, and other mechanically derived conditions.

Help support and align the foot and lower extremities. Do your feet roll inward at the ankle? Orthotics can get you back in alignment.
When are orthotics helpful?
Orthotics can help delay the need for surgery if it is not desired at a specific time. They are also beneficial if a child must wait for growth plates to close and for their skeleton to be more physically mature.
